Project 1
Zimbabwe Palliative Care Awareness Outreach Project
The project started in April 2024 and aims to increase access to equitable PC services and improve the quality of life among PC patients with disabilities and/or elderly patients with life-threatening illnesses in Masvingo
Project 2
SOPP 2
The Supporting Older Persons Project Phase 2 (SOPP 2) is a continuation of the successful Phase 1, funded by an anonymous donor and implemented by Island. This project aims to enhance the quality of life for 5,000 older persons (OPs) in Marondera and Bulawayo, Zimbabwe. SOPP 2 expands the geographical focus to include peri-urban areas.
Project 3
OAK
The project aims to improve health outcomes for PC patients and young carers in Bulawayo, Chitungwiza, Marondera and Mutare. Specifically, the project seeks to:
- To strengthen the capacity of community home-based carers (CHBCs), health professionals (HPs) and religious leaders to deliver PC services in Bulawayo, Chitungwiza, Marondera and Mutare by May 2025
- To provide PC services using relevant models for different patient needs
- To strengthen life skills and community-based psychosocial support for young carers (YC) in Marondera.
Project 4
PCI Phase 4
The project aims to increase access to palliative care especially at primary health care level through capacity building and remote support at primary care level. The intervention is being implemented in Mashonaland East Province targeting HPs and PC patients from a central hospital, a provincial hospital, a district hospital and surrounding rural health facilities. HPs will be trained and mentored on PC while patients will receive direct PC services.
Outcomes of the projects are:
- Improved quality of care for PC patients in MPH, CCH, Mahusekwa and support primary health care centres.
- PC institutionalised at health facilities in all supported Hospitals.
- Improved PC knowledge, attitude, and practice among HPs
- Improved monitoring of PC services
